Global Wellness Clinic Financial Policy
Global Wellness Clinic financial policy is designed to optimize the doctor/patient relationship for the best affordable care possible. The doctors will recommend the care you need regardless as to what your insurance will cover. Our interest is in you. We request that you pay at time of service. This will give you the best value and assist us in keeping your health care cost down. This policy allows us to make monthly wellness visits affordable. We are happy to accept cash, check, debit, or credit card.
We do not accept assignment from Medicare. Reimbursement is usually sent directly to the patient for the services covered by Medicare. For Chiropractors, this is ONLY for manual manipulation of the spine. Please note that Medicare will NOT pay for maintanence care, exams, x-rays or therapies. Medicare or your supplement will reimburse at a % of the allowable fee once the deductible has been met. You are responsible for payment at the time of service. Our office completes the forms for Medicare at no charge.
We do accept traditional Medicaid but do not accept any of the Medicaid HMO's.
Personal Injury or Automobile Accidents
Please notify your auto insurance carrier of your visit to our office and present insurance information to us immediately. Most auto insurances do not pay until the claim is settled, therefore you are still responsible for payment at time of service.
"On the Job" Injury (Worker's Compensation)
If you are injured on the job, your care should be paid for under employer's Worker's Compensation Insurance. You will need to inform your employer of the accident and obtain the name, address of their insurance and their authorization for us to provide treatment. If your employer does not provide us with the information, if a settlement has not been made within 3 months, or if you suspend or terminate care, all fees for services are due immediately.
The policy of this clinic is form payments to be made in full at the time of service. If your child is coming in alone for an appoinment payment should be sent with your child. This policy helps reduce our billing cost which is reflected in our fee schedule. Remember that your child must have a consent form signed if under 18 years of age to be seen without an adult. The Family Plan is $97 and is limited to children who are 21 years old or younger and still living in the home. The plan includes an adjustment and an aqua massage for each member of the family. Insurance will not be filled for the family plan and family should be seen on the same day.
Your insurance coverage is a contract between you and your insurance company and it is your responsibility to know the limits and benefits. We will provide you with the receipts, forms or file electronically when applicable. Understand reimbursement is dependent on your individual policy, and between you and your insurance company.
